Normally potholes happen at underground tunnels where water accumulated after rain, choke drains, many big trucks going by and over short span, top layer begins to breakdown and turn into potholes within days.
Very good example near The Curve Persiaran tunnel, every 2-3 mths hole will appear. After few days you will see a broken bike parked not too far from the hole. Wonder how many life this tunnel has claimed. Again, councils sleeping on job, taking weeks to patch up and doesn't bother to do proper job.

That is another reason. Bad patch up, means hole will very soon appear again.....
