Usually there are 2 main causes of brake failure, 1st is brake fade due to excessive usage of foot brake (mostly happen during long downhill plus vehicle travelling quite fast at that time) and 2nd is when the brake system suddenly not working due to leaking brake fluid, brake lining worn out and master brake pump suddenly 'koyak'. Also, a worn out brake pad/disc rotor will significantly increase chances of brake failure.

Tips/Action to be taken when encounter brake failure apart from fully releasing the throttle and pumping brake few times:
1) Observe the vehicle current/instantaneous travelling speed and decide whether it's safe to downshift one or even two gears because each gear be it manual or auto, except CVT will have maximum speed allowed to be engaged without causing gearbox to burst while manitaining control of the steering wheel (can refer from vehicle owner's manual unless you buy a 2nd hand vehicle). Still, abruptly downshifting and even dipping clutch pedal (for manual cars) is not recommended. Also, NEVER EVER pull up handbrake or engage foot parking brake suddenly to prevent lost control of the vehicle unless the vehicle was slow enough to slowly engage parking brake.
2) Also, gradually steer to leftmost lane (with hazard light turned on) or hard shoulder/emergency lane (if it was available and no obstruction at that particular lane) to bring down vehicle speed safely while using lower gear.
3) Switching off ignition (to 'ACC' position, not 'LOCK' position) is ONLY applicable for a car without power steering AND manual gear (eg Kancil and Viva BX/EX).

Ways to reduce chances if not prevent of brake failure:
1) Observe speed limit and not tailgating/following closely (use 2 seconds rule for normal speed and 4 seconds rule for high speed driving or during bad weather). Even if driving fast also have to see the place 1st and whether it's safe to do so. However, do note that higher vehicle travelling speed requires longer stopping distance as well as higher application of foot brakes due to many satan around that switching lane as they like as well as jahanam that makan angin (drive slow) at middle and fast lane when leftmost lane was perfectly clear.
2) When travelling downhill, lower down a gear or two (depends on steepness of the slope) to enhance engine braking so that foot brake doesn't need to be applied often as well as better control of the vehicle. But, this is not an excuse of not properly maintained the brake system as it is one of the most important lifetime of the vehicle.
3) A more frequent maintance towards brake system should be performed if the vehicle were regularly travelling to long hilly places (such as travelling to Genting and Cameron once every one or two weeks) or frequently stuck in traffic jams and roads with many stop and go traffic.
